Pokemon always has great music. Zelda as a series has phenomenal use of melodies and themes to tie them all together. The Pikmin games have some quirky tracks here an there (I LOVE the Final Trial music, it's, like, creepy and adorable at the same time). Final Fantasy Crystal Chronicles has gorgeous music, thank you Kumi Tanioka. But I'm pretty sure the winner is Shadow of the Colossus. I just, absolutely LOVE it.
Too many ties between games to count. Zelda: Majora's Mask, Pokemon RSE, Skies of Arcadia, Final Fantasy IV, VI, VIII, IX & X, Sonic Adventure, Tekken 2...heck there's probably more but that's off the top of my head.

Ocarina of Time, definitely! Especially Gerudo Valley's orchestral remix, incredible. I love to work it all out on piano to make the songs sound incredible.
Haters gonna hate
That's a hard one, but I'd have to say either Kingdom Hearts II (especially Sanctuary and Dearly Beloved, and I'd be lying if the latter doesn't make me cry a little) or the theme music for both Pokemon R/S/E and LG/FR because of nostalgia. Though, Dragon Quest XII has a pretty amazing soundtrack too, especially the theme for battling Emyprea. <3
It's not possible for me to pick a single, all-time favorite. There are so many great games out there, and a big factor in what makes them great are their soundtracks.
A lot of the ones I like have already been mentioned (Pokémon, Mario, LoZ, Fire Emblem), but I'll draw focus to two specific series: Silent Hill and the games by Team ICO (ICO and Shadow of the Colossus. I have no doubt that The Last Guardian will be included in this...if it's ever released. T_T) Both series do such a wonderful job of creating an atmosphere with their music, I love them to bits.
...oh, let's see. Well, my favorite soundtracks would have to be:

Always liked Pikmin 2 music, the way it adapts. The theme of the wistful wild is also very good (same game)

My favorite game soundtrack would have to be Scott Pilgrim vs. the World the game. Second place would have to go to Sonic Adventure 2: Battle.

I like the soundtracks for the Pokémon games, but that was kinda obvious, XD. In particular, the soundtracks for the Mystery Dungeon games has me under its spell.
Outside of Pokémon, I certainly love the soundtracks for the Elder Scrolls series (particularly Morrowind) and a couple of Final Fantasy games.
As for other games, Fallout 3 had a fantastic sound track, both the original ambient pieces and the hip-happening Dixieland/swing jazz play-lists. Minecraft's sound track is very pretty, but the game doesn't really pick the best song to fit the mood of your current activity very well when it does play music. All of its music is very mellow and chill, and the game itself is not mellow or chill a lot of times.

Sonic CD (JP/EU soundtrack), though I like sonic boom from US soundtrack too.

The Sonic series has some amazing music. It's a toss up between Sonic Adventure and Sonic Adventure 2 (Battle) as my favorites. I also liked Pokemon Colosseum, XD, and Explorers of Sky's soundtracks.
